Current Evidence and Guidelines for Outpatient Prophylaxis

Activity Overview
Venous thromboembolism (VTE) is a common vascular disease and significant public health problem. Deep venous thromboembolism (DVT) and pulmonary embolism (PE) are estimated to occur in up to 2 million Americans each year and are responsible for more deaths than breast cancer, highway fatalities, and AIDS combined. An estimated 300,000 individuals are hospitalized annually in the US for DVT, and it is the number one preventable cause of in-hospital death. Also, VTE results in long term sequelae such as postthrombotic syndrome, chronic thromboembolic pulmonary hypertension, and recurrent VTE.
Given the high rate of outpatient VTE presentation, shortening of hospital stays, and the aging of the population, it is essential that at-risk patients discharged from the hospital receive appropriate prophylaxis. Pharmacists play a key role in the transition of at-risk patients from the inpatient to outpatient setting and need to understand the ACCP guidelines for extended prophylaxis, the risks and benefits of outpatient VTE prophylaxis, and strategies for overcoming barriers to outpatient VTE prophylaxis.
This initiative will focus on strategies for ensuring the safe transition of at-risk patients from the inpatient to outpatient setting, will provide guidance for the safe and effective use of antithrombotic therapy in the outpatient setting, and will present strategies for overcoming barriers to outpatient VTE prophylaxis.

- Describe the ACCP guideline recommendations for the prevention of VTE and the importance of continuity of care for patients transiting from the inpatient to outpatient setting.
- Describe the JCAHO/NQF National Consensus Standards for Prevention and Care of VTE.
- Discuss the role of pharmacists in VTE prophylaxis.
- Identify key elements of an outpatient VTE prophylaxis program.
- Identify barriers to outpatient VTE prophylaxis and implement strategies to overcome them.

The Washington State Pharmacy Association is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education and will issue participants in this activity 1.5 hours CE (0.15 CEU) credit.
